About Danni

Hi there! I’m Danni— healthcare experienced, holder of a Master’s Degree in Social Work, and author of Totally Organized, Even Though I’m Not Here: End of Life Planner.

Leveraging 6 years of social work and case management experience, I help clients overcome healthcare, financial, and social barriers while connecting them to resources that improve outcomes and quality of life.

My professional background includes 4 years in hospital-based case management and 2 years supporting children and families. This experience has equipped me with expertise in care coordination, discharge planning, resource navigation, advocacy, and interdisciplinary collaboration.

In addition to serving private-pay clients, I partner with healthcare organizations, agencies, and businesses as an independent contractor or PRN Case Manager MSW, providing flexible support to strengthen care management teams and enhance client services.
