Humble Case Management helps clients who face a disability, life-threatening disease, health issue, or a social problem. Assist with the achievement of client wellness and empowerment through communication, education, connection of resources, and advocacy.
Many individuals experience barriers pertaining to care and life span due to lack of communication and personal support. By bridging the gap of communication, Humble Case Management Personal Service Coordinator will assist with communication between clinical staff, loved ones, and community agencies to ensure continuity of care and address any questions/concerns.
Humble Case Management mission is to partner with the greater community to increase access to care and appropriate utilization of resources, achievement of optimal health, balanced with the client’s right to self-determination and empowerment. By providing personal service coordination services, Humble Case Management hopes to assist with helping people stay healthy, happy and independent; reducing the chances of problems from arising, as well as supporting people to manage problems as effectively as possible.

iS THERE A CONSULTATION FEE?
No. There is a free 15-minute consultation.

What happens after the completion of consultation?
We will begin working with you and/or your family (on your behalf) to build a comprehensive care plan and provide the service fee. We will keep communication via phone, email, and/or Zoom.
How much does it cost?
Monthly services and non-monthly services are offered, to properly assist your need.
For continuous monthly services, there is a one-time On-Boarding fee of $25.00. Once On-Boarding Fee has been satisfied, services will then be initiated. Payments will be collected monthly to continue services.
For non-monthly services, payment is due at the time services are rendered. Starting at $25 per hour.
Who can use the services?
While I am geared to assist clients with healthcare barriers, anyone is able to complete the consultation to verify if my services meet any other needs you may have.
Can we service clients outside of the leon county and surrounding areas?
Most definitely!
